Medicare Advantage Employer-Group Market Enrollment Trends

Nearly 35.2 million people were enrolled in Medicare Advantage (MA) plans as of November 1, 2025, representing a growing share of the nearly 69 million Americans eligible for Medicare. Individual MA policies, acquired through direct purchase, covered approximately 29.5 million members, as of November 1, 2025. While individual policies dominate the MA market, MA employer-group retiree programs, also known as Medicare Advantage Employer Group Waiver Plans (MA-EGWP, EGHP, or Group MA), continue to experience enrollment increases. Group MA currently encompasses 16.2% of the total MA market.
Employer-group MA enrollment as of November 1, 2025, has grown to over 5.7 million members, an increase of 19,719 or 0.3%, since November of 2024. Furthermore, group market enrollment has grown by approximately 584,000 members since November of 2022. In this brief, Mark Farrah Associates (MFA) analyzes previous year end employer Group MA enrollment reports with recent enrollment reports from the Centers for Medicare and Medicaid Services (CMS).
Group MA Enrollment Trends by Carrier and State
Competition within the Group MA market segment has remained strong as employers, unions, and government agencies continue to move retirees from self-insured Medicare Supplement (Medicare Wrap or Medigap) plans to fully insured MA plans. Overall, the top five companies gained over 363,500 beneficiaries, equating to an 8.3% enrollment increase, over the 4-year period between November 2022 and November 2025 data.
- UnitedHealth has maintained its position as the Group MA segment leader with over 1.8 million members, as of November 2025; a drop of over 122,000 (-6.2%) enrollees since November 2024. During 2025, in Georgia, UnitedHealth lost 51,000 members and 6th place Elevance gained 52,000; and in Alabama, UnitedHealth lost 21,000 members and Humana gained 21,000,
- CVS, the second leading Group MA payer, gained over 7,000 new enrollees since November 2024, an increase of 0.6%.
- Kaiser experienced a year-to-date (YTD) growth of 1.2%, or 7,000 members. The company has continued to see modest gains since 2022.
- Humana continues to show steady growth in employer-group MA enrollment with a 4.4% increase, or 23,234 additional members between November 2024 and November 2025.
- Blue Cross Blue Shield of Michigan’s Group (BCBS of MI GRP) MA enrollment was over 470,000, as of November 2025, a very slight drop from a year ago. Since adding the State of Michigan’s retiree healthcare plan, effective January 1, 2020, BCBS of MI’s Group MA enrollment has grown 3.4% from November 2022.
As per the chart above, between November 2022 and November 2025, the U.S and its territories experienced a 10.8% overall growth rate in Group MA market enrollment. All but four of the leading ten states, by enrollment, experienced membership increases, with California (CA) continuing to top the list at over 751,000 members. Between November 2022 and November 2025, Texas (TX) had the largest percentage increase in Group MA enrollment, 21.3%, representing over 82,000 enrollees.
